Change of Address
It is very important that we hold a correct address and telephone number for you. If either of these change please notify us.
Register with our practice
Patients wishing to register with our Surgery will need to complete a registration form, which you can download or request from one of our Receptionists.
Repeat Prescriptions
Repeat medication is best ordered via the repeat medication service at your pharmacy. Simply ask them to order your regular medications for you every 2 months. If you order medication in this way, you do not need to contact the surgery – simply reorder when you pick up your medication from the pharmacy.
If you do not wish to order your repeat medication via the pharmacy, the next best method is to order your prescription items online. These items are listed on the right hand side of your repeat prescription slip. You will require an online access pin number which can be collected from reception.
If you do not wish to use either of the above methods, we have a prescription line which is option 2 on the main phone number. It is operational between 9:30am – 3:30pm.
Lastly we have a prescription post box which is situated at the entrance to the front door.
We require 3 working days to process any prescription request.
